How To Request Samples
Complete and submit the Sample Request Form to receive your samples. Both the samples and shipping are free of charge. Availability is limited per item, and some products may not be eligible for sampling.

Applications

Applications can be submitted both anonymously and with a real name. If an anonymous application is submitted, a code is assigned to the individual, and they track the process using a password they choose.
If the complaint concerns a member of the ethics committee/subcommittee, that member cannot see it in the system. This aims to prevent negative consequences such as the destruction of evidence or alteration of the investigation process.
